This is where magnets come in real handy. Just add a magnet to the clothing's body part where the poke-thru occurs. Now scale and move the Mag Zone around in order to limit it to the area where the poke-thru is located. Move the magnet around until the clothing covers up the exposed body part.

To make it easier to adjust the Mag Zone, change its display mode to Wireframe:

Make sure you have the Mag Zone selected. On the PC, the shortcut key is Control-Shift-3 to change the display mode of individual objects into Wireframe mode. Or using the menu bar, go to Display -> Element Style -> Wireframe

That'll enable you to more accurately gauge the zone of influence of the magnet as well as giving you maximum visibility of the area in question.
