Many textures on the marketplace are based on photos. But the texture alone is not enough to create realistic skin. The artist probably used Poser 5/Poser 6 shader tricks (did he/she mention something like RealSkinShader or Unimesh Realism Kit?) that add subsurface scattering and displacement to the texture. Some artists are also very skilled at postwork, so that they can take a good render and bring it to a new level of realism. But if you're looking for a good base texture, TamelaJ (3DUnforgettables) and Morris have some excellent textures for sale in the marketplace. They are two of the very best IMO.
