As for the shirt button, I've never seen that happenen. The front placket of the shirt has a little kink in it and looks like you opened each button separately. Make sure you have the shirt selected to Body, not Chest, and then open the shirt by adjusting the dial for the number of buttons you want open (I usually go for the four-button setting for what you're showing here) and you shouldn't get the floating button.
