I start with an established pose eg Mike in erotic or sexy pose on a chair or standing I try to find a pose as near to what I want then I work on that until its done.

And tedious :)  I like to tweak poses, but so many of the poses out there are so far from "normal" that you aren't just tweaking, you are redoing. I personally don't have the patience to do that which is why I use the ones that others make.
