In the novel, and in the remake movie, Alia kills the Baron Harkonnen with her "gom jabbar," a poison needle worn like a thimble on a finger. In the first movie, Alia sticks him with a "gom jabbar", pulls some of the controls for his suspensor units out, and he floats through a breach in the palace wall to be eaten by a giant sand worm (a Freman symbol of cleansing).
The Baron was the illegitamate father of the Bene Gesserit Jessica, Alia's mother. The Bene Gesserit Reverend Mothers possess racial memories of all of their female ancesters up to the moment of conception of the next ancester. They possess no male memories, but the Paul and his children and Alia possess all of their ancestors memories(male and female). The Baron returns from Alia's other memories, and learns of his fate from Alia's memories. He possess her and drives her mad. She committs suicide by jumping out a window. Thus, Baron Harkonnen dies twice.

BTW, if you haven't read "Legends of Dune - The Butlerian Jihad", do yourself a big, big favour and don't bother. It's dreadful. I liked the House Atriedes/House Harkonnen/House Corrino trilogy but the Jihad one is really taking the piss.

I think he introduced some new things even after V4's arrival giving me the impression he's still working on things. I'm just guessing but he might have held off support for the V4 to wait an see. V4 has been in flux for a while. V4.0, V4.1, V4.2...
