That's probably the single most difficult task you could undertake.

You'll need a modelling program of some kind.  There are plenty to choose from, in prices from free to thousands.  At the free end of the scale there are Wings, Blender and Anim8or (and probably several others).  Pricier options are Maya, Lightwave, 3D Studio Max.  Somewhere in the middle there are things like Hexagon.

Then, you have to learn to use 'em.  Each one has a different learning curve.  I'm currently using Wings and it's dead easy to get to grips with, IMO.  Some people can't get on with it, however.

After you've made your model, you have to UV map it.  You'll need UVMapper; there's a free version and a bigger brother, UVMapper Pro, which ain't free.

After that, you'll have to rig the model to enable it to bend in Poser as clothes should do.
