The V3 Body/M3 Head figure is double encoded with RTEjava.

Yes. So it is a custom head? What you need to do if you haven't done it already is Spawn Morph Target. Then REM all morphs on the figure except the one head morph. Then save it to the library.

I don't think  Injection Pose Builder will create the Inj from a custom head.

Try using Staale's "Posers Little Helper" in the Renderosity freestuff instead (this is a PC only app, so I haven't tried it myself, but it looks like it works):
