Did you check the body and hip trans dials?  I did a test where the hip dials were not at zero and I got the same result as in your first image.  Note that Kakadoo's character loads up with settings on the trans dials for the body.

Here's what you should do:

1. Load the character.
2. Make sure it's in zero pose.
3. Select Body. Make sure all trans dials are at zero.
4. Select Hip. Make sure all trans dials are at zero.
5. Export head with default settings.
