Probably because it's not economically viable or marketable.
How would you pitch such a product? Companies can sell their net service as being either super fast or no frills. Where's the marketing spin on middle of the road? Their billing systems may not be set up for 3 types of net service and adding a third, mid range service may cost more than the sales they'd generate in return, since most people tend to want the other options mentioned above.
I guess it's as simple as that.
