Yeah, morkonan I think he's talking about pores and such... which, for close ups, would best be served by a decent image/ alpha based displacement possibly blended with some procedural and rounded off with some masking for good transitions. Image based without burnt in specular, which for nipples and lips is a little difficult to find. Burnt in specular can be remedied (difficulty depending on degree) with the clone stamp and some painting and image adjustments in photoshop.
