Subject: Anyone know if this is possible and if so how to do it?

You can do this using Zbrush or Photoshop/Gimp or even in Poser using the morph tool to just smooth out the features. Depending on how well you manage to smooth it with the morph tools you can always use Gimp/PS to retouch the final image so it looks like the image above.
