We had this before. Now again. Not to be That Person. But can we figure out the root cause and communicate to all who make change to the site code so no accidental overrides?

Considering the number of problems this site's encountered over the years compared to other, larger sites, I think it's a fair observation that there's something fundamentally wrong with the code. Most places don't have these kind of things happen unless there's a problem somewhere else along the line, such as the servers rolling over and dying, as they did with Amazon/AWS a while back, or more recently when Fastly went tits up a couple of weeks ago.
