Would it work to do the sketch in layers as well...one with the outlines completely off, and one with the outlines on, and then take the two results into Photoshop or Paint Shop Pro, place the one with outlines on top of the one with outlines off and erase what you don't want? It's not as sleek as rendering in one step but you wouldn't have the inside the head lines.
