This is M3 SR1 with muscularity morphs. I can not figure out what's causing it, but it's the same with all flavours of M3 - just his Left Shoulder and Thigh are affected.

Anyone seen this before, or have ideas about how to fix it?

Ahhh, this well known bug :)

Go back to Daz and reset your M3 and both his body and head morph packages, then reinstall all 3. Things should work fine after that :)

I could be mistaken but that issue looked like the morphs for those individual body groups were not hooked up to the Fulll Body Morph. These things can happen in text editing or a snafu with the injection.

Either way I had thought the zips for M3 has been updated eons ago to correct this.

Yep, they were updated less than a month after release. It was approximately the end of september 2003.
