What does the exclamation mark inside the blue triangle mean? On a material.

It means that the preview of the material has changed but has not been updated, to get rid of it just click on the preview and it will refresh.

Also, there is a nasty option in the general preferences (preview options: top, right) :
disable automatic material preview rendering

This means that you will always have to click this triangle to get the materials updated. I recommend to uncheck both options in this area.

For some reason, the above option gets activated sometimes, maybe through a keyboard shortcut.

Sometimes disabling material previews (like disabling the mini preview window) can be a useful way of reducing Vue's demand on system resource especially when working with heavy scene.
