Am dyslexic .... need specific instructions on how to find this field/form...just can't find it!! HELP!!! Thanks!

Wait! found it ..... BUT I haven't got the conformation e mail yet!! I want to spend a LOT of $$ on the Dawn/Dusk sale... and will need my special discount coupon!!!

I have a touch of dyslexia myself ... so I've made this as simple as possible. 2 screen-shots.
