As we previously, spoke about, keeping in mind that this is a work in progress, I think you need to turn shadows back on and take care of that rounded blur, if you want to get a nice DOF look. Her face looks a little too washed out, especially with the white/grey hair. I think you need a little more of a vibrant color in there to break up the neutral tones as well. But, all in all, it's shaping up to be an excellent image.

She just pops right out with the way you blurred the bg. Just a little more shadows and it would be perfect!
