Both the Columbia and Challenger accidents would have never happened if NASA's funding for the reusable space boosterplane hadn't been axed. Unfortunately, anti-science troglodytes Walter Mondale and William Proxmire held sway in the Senate. The result, NASA had to replace the safe, infinitely reusable and economical boosterplane with a series of solid fuel boosters that had to be constantly, expensively rebuilt.
