Description
This is a rework of a portrait I uploaded 2 days ago. I tweaked a few elements that only stood out after seeing it "fresh" today. I think this looks significantly more realistic than the previous one.

May I suggest some Ambient Occlusion on the nostrils surfaces to kill the nostril glow. Also note that the texture on the right shoulder has stretched somewhat more than you might want it to (not your fault and pretty hard to fix).

a very beautiful and realistic portrait. The color and reflection of the eyes looks excellent, unfortunately the rest of the eye looks flat - perhaps because the white of the eye is a flat color - no shading, no bloodveins etc. It's the only weakness in an otherwise excellent rendition.

I gotta say this is an astounding image! The lighting is just right, I think. I have to agree with pjz99 above about the shoulder and nostrils. (I've run into the same thing and I'm not sure how to fix either.) So it's only an observation and not a complaint whatsoever. This is truly a VERY realistic portrait. May I ask who the character is and roughly how many lights you used? I find in my images that I feel I get my best results with only one light. I use JoJo and more than one or two lights tend to wash her details out. But I'm still learning too. I got some great helps from BagginsBill her at Renderosity. Check out a thread I started here.
