Charlie Gordon, un jeune homme souffrant d'un retard mental, gagne sa vie comme apprenti dans une boulangerie. Il suit parallèlement des cours de lecture et d'écriture à l'université Beekman avec Miss Kinnian. Un jour, il est convoqué par le Dr Strauss et le Pr Nemur qui lui proposent de subir une opération du cerveau qui doit permettre de démultiplier ses facultés mentales. L'intervention ayant réussi avec la souris de laboratoire dénommée Algernon, les deux scientifiques pensent être prêts à passer au stade de l'expérimentation humaine.
Après l'opération, Charlie, qui est suivi psychologiquement par les deux chercheurs, doit rédiger à cet effet son journal intime sous forme de comptes-rendus. Son ancien professeur, Miss Kinnian, l'accompagne dans son évolution. Charlie Gordon progresse très rapidement, accumule de nombreuses connaissances et maîtrise plusieurs langues, mais a beaucoup de mal à lier des relations stables et normales avec les autres ; faute d'avoir la maturité affective suffisante, mais aussi parce qu'il est obsédé par la compréhension de sa vie antérieure, celle du Charlie Gordon attardé mental, ce qui le conduira à revivre en pleine conscience les scènes les plus traumatisantes de son enfance et notamment ses relations houleuses avec sa mère qui n'a jamais accepté son handicap.
Malheureusement, la souris Algernon donne des signes inquiétants de dégénérescence cérébrale et finit par mourir. Charlie, qui sait fort bien que son sort est lié à celui d'Algernon, comprend qu'il va lui aussi régresser aussi vite que son développement intellectuel a été brutal. Il s'empresse alors de reprendre tous les travaux scientifiques des professeurs Nemur et Strauss afin de trouver les erreurs de calcul permettant d'expliquer l'origine de cette dégénérescence. Il réussit à trouver la cause et rédige un rapport érudit en ce sens. Peu de temps après Charlie sombrera lentement, mais inexorablement, dans l'instabilité mentale de ses débuts, et finira par se résoudre à vivre dans l'asile Warren tant redouté depuis son enfance, laissant l'autre Charlie reprendre son corps.
