I thought I would see the same reaction when Bonnie North was called ma'am in a Star Gate story I did. I was expecting something like " Ma'am ? That's a name for old ladies. Do you see any old ladies here ? I don't. You will address me as Major"

I figure with Starfleet being a bit more relaxed the Ma'am and Sir thing would just go under the usual deference. Probably depends on the officer you're dealing with too.
