You just have to make a mask out of your overlay (ie. black on white mask) and use it with a blender node.
You plug all the diffuse attributes of your skin in the first entry of the blender node, all the diffuse attribute of your overlay texture in the second one, and your black and white mask in the third.
Plug this blender node in the diffuse node.
Do the same for the specular attribute with another blender node. And for all the attribute that are different for the skin and overlay.
As i'm actually rendering, i can't post a pic that would be much more helpful, but i will do asap :)
Here the overlay is different from the skin for the diffuse (the texture), the bump and the specular. You could also add a blender node with the same mask to change the transluscent color or value, and the specular color.

Thanks so much for your picture.
Can I ask you if you could use my set up to demonstrate your way of doing things, I am not that confident with using my set up, it was made for me, it has so many nodes I get confused with it.
I do need my orriginal set up as it is for my favored model. any help thanks

But i tried to do some drawing to explain much.
The highlighted nodes are those i added.
You only have to take into account the nodes that are plugged into the Poser surface. Leave all the rest alone. You have 5 nodes plugged into the Poser surface, so we'll add 5 blender nodes.
For the diffuse. If you don't want the overlay map to be altered by the blue diffuse color you have here, turn the diffuse color into white, and put the blue into the input2 of the blender node.
I put black into the input1 so the overlay will not be altered by the color you have in the ambient color.
Simple here.
I put white in the alternate_diffuse and black in the input1 so the overlay will not be affected by this, only the skin.
Simple here.

Oh, i forget to add:
you have to have the overlay as a separate texture from the skin. A black and white version of the overlay to act as a mask. And for the skin, put your normal torso map.

How about I put it all in a zip file, and email you the material so you can open it in Poser7 and then you can benefit from the file for V4 also if you like. Then you can adjust if for me.

Can anyone tell me how to set up an overlay, on the skin which can be made glossy. I don't want the current overlay which is now part of the skin, as it is affected by the spots nodes and the overlay dose not keep its own textures. but renders showing the spots etc.
I need to keep the current set up so I don't loose the skin spots, tones etc. any help please.
