Subject: Annual Survey freebie

I completed the Annual Survey and chose the Bice & outoftouch freebie however I am on dial-up and the download quit partway through.  When I return to the download page I get this message:

Form Validation Error:

Please go back, correct the errors, and try again.
 

back takes me nowhere.
How can I get back to the download?

The system is showing you have not taken it yet.

You should be able to go in and take the survey so you can get your free download and coupon.
