I'm looking at La Slip-ons 2 and I want to know are the little heart shapes (on the top of the foot/toe region) modeled into the shoe, or are they on a transparency layer so they can be changed to something else?

They're part of the transparency map so you can make your own designs or leave them plain! This is how they load:
