Also, do you have any other lights in the scene? If so what are the shadow settings, intensity, etc?
One other thing (although unlikely) that could be causing this is SSS on another surface other than her skin, such as her hair/clothes.
I have 2012 SR 3.
lights are latexluv IBL HDRI. No other lights
I did add fresnel reflection and I don't think there's a sphere or image. So that could be the problem. I would have to put the sphere there? It doesn't load with anything else?
Forgive me I'm a technical dummy but wher would I see if I had IBL and or IDL turned on/off?

Okay... the reflection isn't showing up in her eyes because if you don't have anything enclosing the scene (such as a room, or a skydome/sphere with an image attatched) then there is nothing for the reflection to reflect. I suggest that if you don't already own a skydome to download BB's EnvSphere HERE.
There are some links on the page to sites that have free panoramic images (it have to be panoramic to work), simply scroll to the bottom of the page and where it says "subpages" click on 'Environment sphere links.'
As for IDL, there's a check box in the render settings. If you didn't check it yourself I doubt it's enabled, so I don't think you have it on. So as far as that goes I'm actually not sure what is causing the black parts. In previous instances, the problem was usually that Firefly would skip over the parts that it had issues with, so for example if Ambient Occlusion (in the Material Room not the lights, IDL automatically deactivates AO on lights but not in the Material Room) and IDL were on, the renderer would have issues with the shadows and would "skip" those parts, leaving black or transparent "holes." But from what you have mentioned, I don't think that's the case this time.

I was already downloading and have the skydome installed amd am running first render. I think I got away wih it before (ie no problems with eye reflection) because all previous renders were with multiple reflective surfaces in the environment? the problems show up with only a colored background and ground as environment.
Thanks for your help thus far. i may not be a technical wizz but I'll stick with it 'til resolved.
I submitted a scene with the black spot problem to SM in the beta testers forum-they are working on it!
