Subject: Does this look like a grouping issue or a mesh issue?

There is a crease under the arm of this outfit I am working on and I'm not sure if this is a grouping issue (the faces I included in the "chest" group) or a problem with the mesh (my modeling?)

Check your joint parameters. It looks as though that area of the chest is not within the blend zones.

Yup, joint parameters - they're tricky in this area. You might want to add some polygons in the armpit, which should make the mesh bend better there: but get the JPs right first so you can see what needs doing.

I looks to me as if you are bending a Shldr and that you have Collars on that outfit. If that is the case then bending the shldr just won't do anything to the chest and what you need to do is extend the groping of the collar so that it goes farther down than it is now.

Like this? If I extend my Collars grouping down? My Shldr is further down the arm, more towards the elbow, so I don't think that has anything to do with it.

How will this, if at all, tie in with the joint parameters? Should I do both - extend the collar grouping and check my joint parameters?

When you say add polygons...do you mean take some of the faces under the armpits and subdivide them so there are more faces so to speak? (I'm using Wings3d) Yeah, my terminology stinks!

I really doesn't matter where the Shldr is located. The shldr can deform all or part of its parent, in this case the collar, depending how the JPs are set up. I would extend the grouping of the collars down, though maybe not quite as far as you are showing . Also you are still going to want to play with the JPs of the Shldrs so that that dont deform the area where the collar and chest meet.

I think Chris is right on this one. The rule is that children can affect their parents, but not their grandparents (true in real life also!)

Or to put it another way, when you bend the Shoulder, the collar moves with it so that the joint remains smooth; but if the chest is required to deform, it can't, and you get a lumpy bit.

The answer is to extend the collars downwards, as you've shown in your second image. If you look at the grouping of other clothes, you'll see that the chest group is quite thin where it meets the abdomen. That way, the longer collar can deform properly when the shoulder bends.

I did mean subdivide, yes; I didn't know what modeller you were using, so I didn't use ANY terminology. :)
