If you look at a wireframe view of V3/V4, you'll see the geometry really doesn't support that very well. Simplest to do it with a bump map imo unless you need those ridges to be visible in profile as standing away from the main surface (then use displacement).
You could probably do the tendons with morphs, but the alien stuff in the
middle would have to be a displacement map. Shouldn't be too hard
to generate that pattern in Photoshop...
