"Alt+click" should work. Make sure you have the right object selected when you go into sculpt mode. If you don't none of the sculpt tools will work. Also on the tool menu there is an add and a subtract button.

Now extrude the edge up and keep extruding and shaping until you get the desired result.

Well this is how I would do it blender.
1st: Creat a cube and squish it down fairly thin.

Well they are both spheres. The UV Sphere is made of quads except at the poles(tris). The Ico sphere is made entirely of tris.

Hey arrow1. I have Howler 10 I have mostly used it for making textures. I have played around witht the terrain thing a bit. Not an expert.It does particle effects and tons of different types of brush effects. I would suggest going on youtube and searching Howler terrain. Also you can buy version 10 right now for $19. You know get your feet wet. You can always upgrade later. They have sales sometimes. Their site is http://www.thebest3d.com/dogwaffle/sales/index.html . Don't know if you are interested but if you are looking for a great paint program I'd suggest Krita. It's free and way cool. https://krita.org/en/
